Your privacy is very important to Catherine Duca and at CatherineDuca.com. Any information you provide on this site such as your email address and other personal identifying information, to the best of Catherine Duca’s knowledge and due diligence will not be shared unless otherwise indicated.
As previously stated, entering this site does not make any individual a client of Catherine Duca. If a user of this site enters into an expressed professional service agreement as a client of Catherine Duca, the following is understood and agreed upon:
I understand my sessions will be held in the strictest confidence, with the following legal and clinical exceptions: If I disclose to Catherine Duca or she determines in her professional opinion that I have ideation with plan and/or intent to harm myself or another individual or an animal, Catherine Duca is required by law as a mandatory reporter, to report such intent and plan to the legal authorities, as well to report any ongoing domestic violence, abuse or neglect of children, the elderly, people with disabilities or animals. It is understood that her legal duty to warn includes, but is not limited to, these circumstances. Catherine Duca will only disclose private information without my consent in order to protect me or any other person or animal from serious harm; for example, if a client discussed plans to commit suicide.
In keeping with the regulations set by HIPAA (Health Insurance Portability and Accountability Act) I understand that all paper records will be kept in a locked cabinet, and any digital or electronic records are password protected.
Cancelled/Missed/Late Appointments
I understand that if I cancel a session with less than 24 hours’ notice, for any reason other than an emergency (defined as an emergent situation or circumstance that would be reasonably regarded as such), I am still required to pay for the session in full.
I am required to phone or send text message to cancel an appointment to let Catherine Duca know even if I have cancelled using an online scheduler.
Where I have purchased and therefore prepaid for sessions packages, I understand that my 24- hour cancelation must be rescheduled by me within a 30-day period, otherwise that session will be forfeited, and I understand I cannot claim reimbursement for it.
I understand it is my responsibility, not Catherine Duca’s, to reschedule an appointment which I have chosen to cancel or change.
Any exceptions to this arrangement can only be made at the discretion of Catherine Duca.
If Catherine Duca needs to reschedule a session with me she will make every effort to give me as much notice as possible and will arrange another time as soon as it is mutually convenient.
I understand it is important for me to be on time for my sessions. I understand that if I am late, the time will be deducted from the total time of the session.
In the event Catherine Duca is running late for any reason she will make every effort to still give me a full session or will add time onto a future session
I understand that if I purchase a package of sessions, they must be used within 6 months of purchase. There will not be a refund for unused sessions in the package beyond that time frame.
